Financial Aid Process
For the 2024-25 Year:
Please login into MyRVC/Self Service and visit your Financial Aid home page to view your required documents. To ensure timely processing of your financial aid, please kindly submit all requested documents and forms together. This helps us provide you with the best assistance possible.
For the 2023-24 Year:
- Review the forms requested, sent to your student email from the Financial Aid Office. If you can't find this message, you can see which forms are missing via Self Service, under the Financial Aid tab.
- Complete Form 1 and any other requested forms as noted in your Financial Aid email or within Self Service.
- Promptly return all requested documentation to the Financial Aid Office. Please note: We are unable to accept pictures of forms/documents.
- Check your Self Service account regularly for any updates or additional forms/documentation requests. The Financial Aid Office will reach out to your student email if additional information is needed in the review process.
Submit all requested forms and documentation by May 1 to ensure a Financial Aid Award Letter in time for Fall Semester.
Financial Aid Forms
- 2023-2024 Forms are required for Fall 2023, Spring 2024, and upon student eligibility Summer 2024 semesters.
- 2024-2025 Forms are located within MyRVC/Self Service. Please review the instructions at the top of this page.
Form 1: Financial Aid Information Form
All students seeking Financial Aid at Rock Valley College must complete this form. This is a six-page document. Please print, read, sign, and return all six pages to the Financial Aid Office. This form explains in detail the academic standards all financial aid recipients must maintain in order to receive future aid as well as the applicant's rights and responsibilities, and other important information.
Forms 2-17: Verification Forms
Only complete the forms listed below if they are requested of you. A list of requested forms will be sent to your student email. You may also review them via Self Service, under the Financial Aid tab.
The forms below are required if your FAFSA form was selected for verification by the Department of Education, or if after review by the Financial Aid Office more information is required to resolve conflicting information within your file.
- You are considered Independent if you were not required to provide parental information on the FAFSA.
- You are considered Dependent if you were required to provide parental information on the FAFSA.
Form 2: Independent Standard Verification Form
Required for independent students in the standard verification group. Requires students to provide information such as household, work, and income information.
Form 3: Dependent Standard Verification Form
Required for dependent students in the standard verification group. Requires students and parents to provide information such as household, work, and income information.
Form 4: Independent Custom Verification Form
Required for independent students in the custom verification group. Requires students to provide information such as high school completion status.
Form 5: Dependent Custom Verification Form
Required for Dependent students in the custom verification group. Requires students to provide information such as high school completion status.
Form 6: Independent Aggregate Verification Form
Required for independent students in the aggregate verification group. This form requires students to provide information such as household, income, and high school completion status.
Form 7: Dependent Aggregate Verification Form
Required for dependent students in the aggregate verification group. Requires students and parents to provide information such as household, income, and high school completion status.
Form 8: Illinois Residency Verification List
Provides a list of documents approved by the state to verify Illinois residency for the Illinois state MAP grant.
Form 9: Curriculum Code Form
Consists of a list of all programs that are eligible for financial aid. Form is required if you do not have an eligible code recorded in the RVC Records Office.
Form 10: Independent Income Verification Form
Students and parents may be asked to fill this form out if income reported on the FAFSA appears to be unusually low.
Form 11: Dependent Verification of Income Form
Student may be asked to fill this form out if income reported on the FAFSA appears to be unusually low.
Form 12: FAFSA Signature Form
Students and parents (if student is dependent) will receive this form if they did not sign the FAFSA electronically or did not submit a paper signature to the Department of Education.
Form 13: Enrollment History
Students are required to complete this form based on prior college enrollment.
From 14: Student Statement
This form may be required when additional information is requested in the form of a statement.
Form 15: Dislocated Worker Verification
Students which fall in the dislocated worker category for verification of the Map Grant will be required to complete this form.
Form 16: Standards of Progress Appeal Form
Students receiving financial aid must make Satisfactory Academic Progress (SAP) toward the completion of course requirements in a certificate or degree program. Fill out this form if you have been notified that you are no longer meeting the standards for financial aid and wish to appeal due to circumstances beyond your control.
Form 17: Permanent Standards of Progress Appeal
Per federal regulations, there is a limit to the amount of time a student may receive financial aid for their educational program of study. If you’ve received notification that you have exceeded this limit, and you have extenuating circumstances as to why you have not completed your program in the allotted time frame, this form may be utilized to appeal this limit.
The Department of Education mandates that colleges obtain specific documentation when verifying information that students submit when applying for financial aid. Rock Valley College also requires that students read their Rights and Responsibilities and the Standards of Academic Progress policy. Depending on your individual circumstance, additional information may be necessary to process and award your financial aid.
